Redesigning Primary Care Delivery at Loma Linda University Health

In partnership with Nudj Health, Loma Linda University Health launched a technology-integrated whole person care initiative to address some of the most significant challenges facing healthcare today.

Designed as an operational extension of primary care itself, this initiative combines Nudj Health’s lifestyle medicine, behavioral health, longitudinal patient engagement, connected monitoring, and analytics platform directly within Loma Linda University Faculty Medical Group Department of Family Medicine clinical workflows and Epic electronic medical record infrastructure.

In practice, Epic automatically flags eligible candidates before their appointments, allowing a physician to order lifestyle services within seconds during a routine visit. The referral connects patients with a dedicated care team of health coaches, dietitians, and behavioral health specialists who help them act on the doctor's prescription. Regular data tracking and compliance reports then flow directly back into the provider's inbox for seamless oversight and billing.

The initiative went live on May 14, 2026 and immediately demonstrated strong clinician adoption, generating 62 patient referrals in its first four and a half days. By utilizing Medicare-reimbursable CPT codes, the model addresses the financial barriers that traditionally stall lifestyle medicine programs.

With funding from Ardmore Institute of Health, ongoing research will evaluate clinical outcomes, healthcare utilization and cost trends, patient engagement and satisfaction, provider experience and satisfaction, and financial sustainability associated with caring for Original Medicare populations using this model.
