Carilion Clinic: A Model for Primary Care-Based Lifestyle Medicine

Carilion Clinic, based in Roanoke, VA, recently published, Lifestyle Medicine in Action: A 2-Year Evaluation of a Primary Care-Based Clinic in a Safety Net Population. With grant support from Ardmore Institute of Health (AIH), the Carilion Clinic team evaluated its primary care-based lifestyle medicine clinic two years after its inception. The research suggests, “A primary care-based lifestyle medicine clinic shows potential for improving patient outcomes in this safety net setting; broader impact requires improved access, support, and tailoring to context.”

Carilion Clinic also created the Lifestyle Medicine Clinic and Registry Toolkit, a practical guide for embedding lifestyle medicine services directly into primary care settings. The toolkit, designed to support other organizations in establishing a lifestyle medicine clinic and/or data registry, provides:

  • Electronic health record documentation templates for individual patient visits and shared medical appointments

  • Framework for building a patient registry, including initial data collection protocols and a complete data dictionary for tracking patient outcomes and quality improvement metrics

  • Practice-ready patient education materials

  • Background on Carilion Clinics’ development process to help other systems launch their own clinic model
